Introduzione: in Pronto Soccorso, l’ambiente caotico e sovraffollato, le condizioni critiche del paziente e la criticità del fattore tempo sono alcuni degli elementi che possono contribuire ad aumentare il rischio di errore da parte degli operatori, con ovvie ripercussioni negative sul paziente, sui professionisti e sull’organizzazione. In questo lavoro si presenta l’applicazione della tecnica di analisi proattiva Fmea/Fmeca (Failure Mode and Effects Analysis/Failure Mode, Effects, and Criticality Analysis) nel Pronto Soccorso di Faenza.
