Retrospective study on the pre-hospital management of acute myocardial infarction in the province of Piacenza and 30-day mortality analysis: focus on nurse-led teams

Introduction: in Italy the pre-hospital emergency system varies in terms of organisational models and team composition. The most widespread model, as in Piacenza, is the ‘hybrid’ model. Primary objective: to describe the management of patients with STEMI with focus on nursing and medical leadership teams. Secondary objective: to analyse 30-day mortality.

Materials and Methods: the present study is a retrospective, descriptive, single-centre study conducted over the three-year period 2021-2023. Data were extracted from the STEMI registry. Parametric and non-parametric tests were used to analyse the following variables: ECG performance, therapy administration, severity codes and mortality.

Results: a total of 292 patients were included, the majority managed by nursing-led teams (58.9%). There was no statistically significant difference in mortality between nursing and medical teams (p=0.198). A significant reduction in mortality was observed in patients who received complete therapy according to PDTA (p=0.011).

Discussion: nursing care is confirmed as safe and effective, with outcomes comparable to those of the medical teams. Adherence to PDTA is associated with a significant reduction in mortality.

Conclusion: the hybrid model adopted in Piacenza proves reliable in the pre-hospital management of patients with STEMI.
