Scopo: Analizzare la percezione degli infermieri di pronto soccorso che assistono i pazienti psichiatrici con atteggiamento aggressivo,per comprendere come strutturano la gestione dell’evento violento.
Materiali e Metodi: Disegno di ricerca: studio qualitativo fenomenologico. Campione: 18 infermieri di due dipartimenti di emergenza. Intervista semi strutturata e trascrizione integrale delle interviste. Le interviste si sono svolte tra il 28 giugno e il 2 agosto 2016. L’analisi del contenuto: metodoVan Kaam.
Risultati: L’assistenza ai pazienti psichiatrici aggressivi causa negli infermieri emozioni quali: paura, tristezza, sofferenza, rabbia, disagio e la sfiducia nel paziente stesso a causa della sua imprevedibilità . Sono stati identificati sei temi: definizione e idea della violenza; segni che preannunciano i comportamenti violenti; vissuto degli infermieri nella gestione dei pazienti psichiatrici aggressivi; esperienze di violenza; reazioni emotive degli infermieri dopo un evento di violenza; aspetti di assistenza infermieristica del paziente psichiatrico aggressivo. Emerge l’importanza di mantenere un atteggiamento accogliente e calmo, di utilizzare uno stile comunicativo appropriato di fronte all’escalation di un paziente aggressivo e ciò favorisce la de-escalation del paziente. La maggioranza avverte necessaria la partecipazione a corsi di formazione relativi all’aggressività del paziente psichiatrico.
Conclusioni: Gli stati d’animo dell’infermiere che si trova ad assistere questa tipologia di paziente possono contribuire a una gestione inefficace sia dei pazienti che degli eventi. L’insicurezza o l’incapacità nel prendersi cura di un paziente psichiatrico possono dipendere sia da una mancata preparazione professionale, sia dall’iniziale approccio che ogni operatore instaura con il paziente aggressivo che, se non adeguato, può implementare l’escalation del paziente.
