Introduzione: l’aumento progressivo del fenomeno dell’obesità a livello mondiale e nazionale indica che le persone affette da obesità morbosa (classe III° della classificazione OMS) si rivolgeranno in numero sempre maggiore ai servizi di emergenza e alle cure ospedaliere per fatti acuti.
Le caratteristiche peculiari dei pazienti cosiddetti bariatrici impongono, da parte degli infermieri, importanti accorgimenti di tipo clinico, logisticoed organizzativo, a partire dall’accoglienza in pronto soccorso (PS).
Materiali e metodi: revisione narrativa della letteratura internazionale.
Risultati: le dotazioni logistiche per la gestione del paziente con obesità morbosa in PS non sono sempre adeguate alle reali necessità. L’impatto delle persone obese sui carichi di lavoro non è attualmente ben chiaro, anche se sembra che l’obesità grave implichi un tempo maggiore di permanenza in PS. In generale, gli obesi con lesioni traumatiche sperimentano maggiori complicanze e mortalità rispetto ai normopeso. I pattern di lesione traumatica di questa popolazione si caratterizzano per un numero significativamente minore di traumi cranici, una maggior quota di traumi toracici e di fratture delle estremità inferiori. Si rilevano difficoltà nella gestione delle vie aeree, nella pre-ossigenazione, e nella gestione della ventilazione con pressioni protettive (p-plateau ≤ 30 cmH2O). Il reperimento di vene periferiche può essere complesso e richiedere cateterismo
venoso centrale con guida ecografica. L’esecuzione e l’interpretazione di ECG a 12 derivazioni è influenzata dal pannicolo adiposo presente a livello toracico. La misurazione della pressione arteriosa richiede bracciali di misura adeguata. I dosaggi dei farmaci devono essere prescritti sulla base del grado di lipofilia. L’esposizione ed i trasferimenti risultano problematici, e la cute ad alto rischio di lesioni da decubito, e dovute a devices intrappolati nelle pieghe cutanee. La diagnostica strumentale può essere fortemente limitata.
Conclusioni: a oggi è necessario implementare la ricerca in ambito clinico ed organizzativo per ottimizzare l’assistenza verso questa categoria di persone.
