Introduzione: l’umidificazione ed il riscaldamento dell’aria inspirata, sono compiti normalmente assolti dal tratto respiratorio superiore. Con l’intubazione endotracheale e la tracheotomia si bypassa il tratto naso-orofaringeo, le regioni del corpo con più alta capacita di riscaldamento ed umidificazione. Vengono inibite le difese naturali quali lo starnutire, il tossire, il deglutire. Il sistema di trasporto muco ciliare resta l’unica difesa meccanica ma l’esposizione a gas freddi e secchi lo rallentano e immobilizzano.
